  Notes	A Form of Statutory Declaration for the correction of an error of fact or substance in a Register of Marriages was completed by Ivy Upton nee Saunders of 105 Kentish Town Road, London NW1 at East Finchley on 13 March 1951 before Col. Sir Edwin King, KCB, CMG, JP for Middlesex of The Old House, East Finchley, London N2. The declaration reads: I ... do solemnly and sincerely declare that Violet May Jackson, who was married to Albert Frederick Minter on 14th July 1929 at St Benet and All Saints Church, St Pancras, is one and the same person as Violet May Forman whose birth on 2nd December, 1904 at Kingston, Surrey, was registered as the daughter of Herbert Forman and Rosina Forman, formerly Saunders. So far as I am able to recall, the man Herbert Forman (my step father) left his wife Rosina Forman (my mother) just before the birth of their above-mentioned daughter, Violet May, at which I was present. It was after I had been sent away to domestic service a few months later that the man Joseph Jackson joined my mother and remained with her until his death some years ago. They lived together as man and wife, my step-sister naturally coming to regard him as her father, but I have no evidence of their marriage, if they ever did marry.
